Guidance on Respiratory Illness

In accordance with , the University policy for respiratory illness (eg, influenza, COVID-19, RSV, etc.) is as follows:

If you feel sick stay home—employees should inform their supervisor, and students should inform their faculty, when they cannot be present on campus due to illness.
All employees and students can return to their normal activities and return to campus when, for at least 24 hours, both are true:
- Your symptoms are getting better overall, and
- You have not had a fever (and are not using fever-reducing medication).
When you return to your normal activities, take added precaution over the next 5 days, such as taking additional steps for , , , and/or when you will be around other people indoors.
- Keep in mind that you may still be able to spread the virus that made you sick, even if you are feeling better. You are likely to be less contagious at this time, depending on factors like how long you were sick or how sick you were.
- If you develop a fever or you start to feel worse after you have gone back to normal activities, stay home and away from others again until, for at least 24 hours, both are true: your symptoms are improving overall, and you have not had a fever (and are not using fever-reducing medication). Then, take added precaution for the next 5 days.

Immunization:
The CDC recommends everyone stay up to date on immunizations for respiratory infections, and the University strongly encourages faculty, staff and students to consult with their health care provider and to stay up to date with all recommended vaccines, including the COVID-19 and influenza vaccines. Note that some programs require students to have additional vaccines, like COVID-19 and/or influenza vaccine, based on requirements of experiential sites. Check with your specific program for requirements.

Immunization Policy

Students are required by Illinois law to have an immunization record on file if they were born on or after January 1, 1957, and are planning to enroll in six or more credit hours at any time during their attendance at �ؾ��. This record must contain proof of immunity to measles (rubella –must have received one dose on or after the first birthday), mumps, rubella, diphtheria and tetanus (the last T/D booster must be no more than 10 years old).

A second measles (rubella) immunization with live attenuated virus vaccine must have been given 28 days or more after the first measles immunization. If under the age of 22 the state of Illinois requires to show proof of one dose of Meningococcal conjugate vaccine on or after 16 years of age.

Some programs within the university may require proof of additional immunizations, such as annual influenza or COVID-19, based on requirements of clinical training sites. Students should check with their program for more information. Additionally, the university encourages all students to stay up to date with immunizations including annual influenza vaccine and COVID-19 vaccine, and are encouraged to consult with their health care provider.

Students who do not have immunization records to verify these vaccinations should immediately contact their physician to obtain a copy of their records or have the appropriate immunizations administered.

All eligible students must have a current immunization record on file before the end of their first semester at �ؾ��. The immunization records will be kept on file for 10 years from the first semester of attendance.

General Instructions and Information (Compliance is Mandatory)

�ؾ�� has partnered with Med+Proctor for immunization compliance and verification. We no longer accept proof of immunization via mail, fax or email. Med+Proctor is an electronic medical platform where you can upload your immunization records. Med+Proctor is also HIPPA and FERPA compliant and your confidential information will remain secure.

- NOTE: Med+Proctor has an option for lifetime medical records storage. You may choose this option for a one-time fee of $10 by selecting the green button. To submit your forms for free and decline lifetime storage, please select the grey button.
If you cannot secure a copy of your records, make an appointment with your physician or other health care provider to complete the immunization form. All information must be in English. Once complete, the form can be uploaded to Med+Proctor.
Illinois high school or college immunization records are acceptable if they contain all necessary information on the required immunizations and are properly certified.
Anyone on an approved schedule to receive all necessary doses of vaccine must indicate the date of the first and the expected dates of the last two remaining doses.
A physician, school/college/university health service, registered nurse or public health official must certify all dates by signature and must include his or her address and phone number. Month, day and year must be listed for all dates.
Any laboratory or radiological evidence submitted by students must include copies of any reports showing their names and dates of tests and results.

Exemptions

Only the following exemptions will be accepted if they are supported and properly verified by statements that accompany the Certificate of Immunity form:

Medical Contraindications: Students can submit written, signed and dated statements from their physician specifying the vaccines that are contraindicated and the duration of the medical condition(s) that contraindicates them. If such contraindications are temporary, students must then comply with the immunization policy as soon as their physician approves.
Pregnancy or Suspected Pregnancy: A signed statement from a physician verifying pregnancy or suspected pregnancy will temporarily exempt a student from this requirement. The physician's statement must indicate the expected date of delivery. After delivery, the student must then comply with the immunization policy as soon as her physician approves.
Religious Exemptions: Objections to immunization on religious grounds must be corroborated by a letter stating that such objections are based on bona fide religious tenets or practices. The letter must be signed by the student or parent.

Insurance

It is �ؾ�� policy to provide proof of health care insurance for students living in the residence halls, participating in athletic programs, studying in the Pharmacy program or Doctorate in Clinical Psychology program as well as all F1 and J1 international students. Emergencies

In the event of a serious emergency, students should call 911 or have someone take them to the nearest emergency room. The student should show his or her insurance identification card to inform the emergency room personnel of coverage for emergencies.

SMOKE-FREE ENVIRONMENT

The Illinois Clean Indoor Air Act became effective on July 1, 1990. This act applies to �ؾ�� as it applies to "any enclosed indoor area used by the public or serving as a place of work including, but not limited to, restaurants, retail stores, offices elevators, indoor theaters, libraries, concert halls, educational facilities, auditoriums, arenas and meeting rooms." Under this Act, �ؾ�� must prohibit smoking in all of its public areas, on all campuses, including lobbies, libraries, lounges, bathrooms, and in all of its work areas and conference rooms. In addition to this prohibition on indoor smoking, the Chicago Clean Indoor Air Ordinance that became effective January 16, 2006, prohibits smoking within 15 feet of the entrances to �ؾ��'s Chicago Campus buildings. Fines and other penalties are assigned for non-compliance.

No cigarettes or other tobacco products will be sold on University premises.
