Congratulations on your acceptance into the 藏精阁 Doctor of Pharmacy program! Welcome! This checklist contains the remaining items you need to complete and mail to us prior to your arrival.
Electronically submit your Matriculation Agreement, Merit Scholarships Terms, Conditions & Acceptance Letter, and Technical Standards for Admission, Promotion and Graduation (link sent in acceptance letter).
You will receive information through PharmCAS regarding your criminal background check and drug screening. If you have any questions about this process or the status of your screenings, please contact the enrollment office at pharmacy@roosevelt.edu or call 847-330-4500.
Final transcripts for pharmacy prerequisite courses or completion of a degree must be submitted to the Pharmacy Office of Enrollment & Student Services by May 31 of your admission year. Transcripts must validate satisfactory completion of all prerequisite pharmacy courses taken since the submission of the application for admission, and the minimum cumulative grade point averages required by the College. All students must acquire an IL PharmTech Student License from Illinois Department of Financial & Professional Regulation through the website: . The license must be obtained and submitted to the Office of Enrollment and Student Services by May 31 of your admission year. It will be renewed annually, by March 31.
Due to requirements of experiential practice sites, pharmacy students are required to attain current health insurance throughout their time in the program. Students will be required to provide proof of insurance at least annually.
An Immunization History form is required to complete your conditional admission. Download the Immunization History Form and take it to a primary care physician for completion, and then return it to the Pharmacy Office of Enrollment & Student Services by the date specified in acceptance documents. Don't forget to keep a copy for your records. Completed immunization records must be on file before the student will be allowed to attend classes.
Admitted Students Day occurs in the spring before your first summer term. It is an opportunity to connect with your future classmates, learn more about financial aid, housing and what to expect in your first year in the Doctor of Pharmacy program. Attendance is optional but strongly encouraged. You will receive an invitation with more information in March.
New Student Orientation is mandatory and occurs on the first day of your summer term, Monday, June 16, 2025. You will be meet your classmates and your faculty mentor, receive your laptop computer, meet faculty and staff, receive your summer term course schedule, and complete any last-minute enrollment details..
