We have several ways in which a student may apply to our Doctor of Pharmacy (PharmD) program at ²Ø¾«¸ó. Below will identify the application process and eligibility requirements.

Regardless of the application pathway, all students will:

1. Participate in an interview.
2. If recommended for admission by the Faculty Admissions Committee,
   1. Accept the offer and submit a non-refundable admission deposit which will be credited to the student’s first term in the program.
   2. Agree to the pre-matriculation documents: Matriculation Agreement, Scholarship Merit Terms and Conditions Acceptance, and Technical Standards for Admission, Promotion, and Graduation.
   3. Successfully pass a Certiphi Criminal Background Check and Drug Screening (Admission is contingent on satisfactory results on this Criminal Background Check and Drug Screening).
   4. Submit a complete Immunization History Form.
   5. Maintain health insurance while enrolled as a student.
   6. Possess and maintain an active Illinois Pharmacy Technician License while enrolled as a student.
3. Successfully complete the required prerequisites by June 1.
4. Submit final official transcripts that validate satisfactory completion of all prerequisite pharmacy courses taken since the submission of the application for admission, and the minimum cumulative grade point averages required.

In order to select the strongest candidates from the pool of qualified applicants, the College of Science, Health & Pharmacy Admission Committee will assess not only the successful completion of a pre-pharmacy curriculum, but also the letters of recommendation, scores from the personal interview, PCAT score if submitted, volunteer and co-curricular experiences, health-related work experiences, and written communication.

Applicants selected for admission into the Doctor of Pharmacy program will receive a conditional offer of admission. Admission offers are made until all program spaces are filled.

Offers of admission are conditional until all outstanding requirements are completed as outlined in the program’s Matriculation Agreement.

Vaccination series and any booster doses as per ACIP guidelines are requirements of the Doctor of Pharmacy program without the option for exemption, due to the clinical site requirements needed for completion of program requirements.
