Pharmacists in the ambulatory care setting can serve patients through comprehensive medication management skills in order to further enhance chronic disease state management.

Serving in the outpatient cardiology setting, the majority of patients that require intensive care are those who recently suffered heart attacks and those who live with chronic heart failure and require medication therapy management.

Smoking is the number one preventable cause of death worldwide. Utilizing skills like motivational interviewing and with the knowledge of pharmacotherapy, pharmacists are at the frontlines of preventing smoking-related health effects. This is also a topic of current research; I am investigating smoking cessation rates within an outpatient Cardiology Clinic. The smoking cessation program is pharmacist-lead and phone-based.

Diabetes care requires careful medication therapy management and patient empowerment through education and practicing self-efficacy skills. With their knowledge of pharmacotherapy, pharmacists can serve as diabetes care and education specialists.

With the expertise in pharmacotherapy, pharmacists can employ comprehensive medication management in order to serve as chronic disease state management experts.
